This picture is a powerful visual metaphor. It predisposes you to feel a certain way, because the image looks like someone being hanged to death. Death invokes visceral emotions of disgust and sadness. Whether or not you are anti-smoking, this image provokes powerful feelings that lure you into the anti smoking campaign. the image shines light on the fact that we are in control of the damages of cigarettes. the add goes as far as to infer that death by cigarettes is suicide. This puts the viewers in the point of control over their own individual decisions. This image is different from others, because unlike adds that address a consumer as a whole, this image calls upon the consumer as an individual.
